Testicular torsion is twisting of the spermatic cord, which cuts off blood supply to the testicle. It is a urologic emergency because irreversible testicular damage can occur within hours if not detorsed. Typical presentation is sudden, severe unilateral scrotal pain with swelling; the testis may sit high and horizontally. The cremasteric reflex on the affected side is often absent. Doppler ultrasound can help by showing reduced or absent blood flow, but high clinical suspicion warrants immediate surgical detorsion rather than waiting for imaging. Risk factors include anatomical variants such as a bell clapper deformity. The other terms refer to unrelated conditions: fainting, miscarriage, and a sexually transmitted infection.
